Visitor Connection Form — Terms of Service

By submitting the Christian Development Center Visitor Connection Form, you acknowledge the following terms.

1. Voluntary Participation

Providing your information is voluntary. You are not required to complete this form to attend worship services or participate in general church fellowship.

2. Purpose of the Form

The form is intended to help CDC welcome visitors, respond to requests, provide information about church activities, and offer appropriate spiritual support.

3. Accuracy of Information

You agree to provide information that is accurate to the best of your knowledge and that you are authorized to share.

Please do not submit another person's private information without appropriate permission.

4. Communications

Submitting the form permits CDC to contact you as reasonably necessary to respond to your visit, inquiry, or specific request.

Optional recurring church announcements, newsletters, and text-message updates are subject to your separate communication preferences and any legally required consent.

You may withdraw your permission for optional communications at any time.

5. Spiritual and Pastoral Support

CDC may offer prayer, biblical encouragement, and pastoral guidance as part of its religious mission.

Such support is spiritual in nature and is not intended to replace licensed medical, psychological, legal, financial, or emergency services.

Submitting a prayer request does not establish a professional counseling or other licensed professional relationship.

6. Privacy

Personal information will be handled according to CDC's Privacy Policy and applicable law.

7. Limitations

CDC makes reasonable efforts to respond to visitor inquiries but does not guarantee a particular response time, outcome, or availability of requested services.

To the extent permitted by applicable law, CDC is not responsible for interruptions or failures of third-party form services outside its reasonable control. Nothing in these Terms excludes liability that cannot lawfully be excluded.

These Terms are governed by applicable laws of the State of Florida and the United States.
